"Human-in-the-loop" is how the industry says "don't worry, it's safe."

It appears in every governance deck and vendor FAQ as the control that makes autonomy acceptable. It is comforting, board-friendly, and rarely examined. That made it exactly the assumption worth pressure-testing in public.

A human in the loop is a control that gets weaker as you succeed.

The point of automation is more decisions than a human can review.

Scale and meaningful human review are in direct tension. The more the system does, the thinner the human's attention is spread across it.

Under volume, "approve" becomes a reflex, not a judgment.

Ask a person to sign off on hundreds of actions and the loop degrades into a formality. The control exists on the org chart, not in reality.

It moves accountability without adding safety.

A loop that can't actually catch the bad action still names a human to blame for it. That is governance theater, not a safeguard.
